Any area is subject to decline by leader, director or pastor over that area.
if your request is for an ongoing use of any areas please select yes and the start date and times. then in the final comments let up know any other dates you would like to request (up to 6 months)
Please select a whole hour time frame, please allocate time for set up, tear down and clean up of any and all areas requested.
* If you would like to reserve more that 3 hour slots, please use "other" and specify times (H-H)
If you are requesting multiple dates, please list all the dates here (every Monday). If you are requesting more than one day per week (Monday & Friday), you will need to complete a separate form for each day. Be sure to also include your reason for the request in the space provided.
